If you are at high risk for Type 2 diabetes
You get accees to a 12-month online program, a FREE connected smart scale, automatic tracking for weight and steps, CDC-approved lessons, and expert coaching.
Enroll if you are 18 years or older, have a BMI of 25 and above, have no history of Type 1 or Type 2 diabetes, and your blood tests are in the prediabetes range or you have a history of gestational diabetes.

How do I sign Up?

“Let’s start with the basics” and enter information about the person who will be using Livongo.
Enter PREMERAWA under “Who has you covered”. To confirm coverage, choose Premera Blue Cross and enter your Premera member ID and group number (see your Premera ID card).
Answer a few questions to “Find the Livongo Program for You”.
If you qualify, “Set Up Livongo to Work for You” by selecting the preferred contact method.
