VISION

so you can see clearly

The vision plan covers one routine eye exam per calendar year and offers a vision hardware benefit every other year. You are covered through Premera.

Eye exam: You pay a $25 copay when you see an in-network Premera provider.

Vision hardware: You can use up to $200 for vision hardware, including prescription lenses and frames for prescription glasses, prescription contact lenses, prescription safety glasses, and prescription sunglasses. Your $200 vision hardware benefit resets each odd calendar year.


enrollment

who can enroll?

You - if you are a regular and active employee and you work a minimum of 30 hours per week or 130 hours per month. If it’s unclear how many hours you will be working per week at the time of hire, you may qualify later. Find out more about who can enroll here.

Your family - your spouse or qualified domestic partner, your dependent children up to age 26 or longer if your child is disabled. Learn more about who can enroll here.

DO I HAVE TO BUY VISION INSURANCE?

No. You have the option to buy vision insurance for yourself and your family. BUT, you must buy vision insurance for yourself if you want to enroll a family member.

using the benefit

when does my benefit kick in?

You can get an eye exam and use your hardware benefit the day you start work or when you first become eligible for coverage.

if I don’t use my hardware benefit, will lose it?

Your vision hardware benefit resets every odd year, regardless if you have used all or some of your hardware benefit.
